The Madlanga Commission of Inquiry has raised serious questions about the origins and handling of an investigation into Crime Intelligence head, Dumisani KhumaloLieutenant-General Dumisani Khumalo after an IDAC investigator was unable to explain how the probe was initiated or what prompted it.

Testifying before the commission on Wednesday, IDAC investigator Colonel Brian Padayachee faced sustained questioning from commissioners over the investigation into Khumalo's vetting and security clearance, as well as the inclusion of several other senior SAPS officials in the matte

The commission heard that Padayachee relied on information passed on by other officials but could not account for the original referral that triggered the investigation.

Commission chairperson Justice Mbuyiseli Madlanga expressed concern that investigators appeared to have proceeded without first establishing the basis of the complaint.
