Campaigner Lisa Squire, whose daughter was murdered aged 21, says 'we're not learning from previous incidents' after Levy was convicted of killing two vulnerable women18:31, 08 Aug 2026The mum of murdered Libby Squire has branded missed chances to stop a serial sex predator who killed two women “a catalogue of disaster”.A string of opportunities to catch Simon Levy, who murdered two vulnerable females and left a third for dead, were squandered by authorities. Campaigner Lisa Squire says she recognises the pattern of offending from the case of her daughter, who was raped and murdered in 2019, aged 21 after disappearing following a night out with friends.Her killer Pawel Relowicz had earlier committed a string of sexually-motivated crimes.Mrs Squire said: “We’re not learning from previous incidents. This is just a catalogue of disaster, isn’t it?” Levy, 40, faces a whole-life sentence after being convicted on Friday of murdering gran Carmenza Valencia-Trujillo, 53, in March 2025 and mum-of-four Sheryl Wilkins, 39, in August that year.He was also convicted of rape, grievous bodily harm with intent and the non-fatal smothering of a third woman. It was part of a seven-year spree that included multiple sex attacks while the police and courts failed to stop him.Levy was repeatedly released on bail or not arrested despite failing to attend court hearings for other offences, and leads were not pursued until after he had killed twice. Asked if she recognised the pattern of offending from her daughter’s story, Mrs Squire said: “Yes, I mean the patterns were there. He was flying those red flags."The offences he was committing on public transport and what have you. These offences are precursor offences and we need to be looking at those far more seriously because if we understand those offences then hopefully we would have been able to go forwards and stop what happens at the end with the death of those two young women.”Article continues belowIt comes as London’s Victims’ Commissioner Andrea Simon said: “It is an absolutely horrific case and I’ve been appalled at the litany of failures to manage such a prolific offender.”On Friday, British Transport Police (BTP)’s Assistant Chief Constable Charlie Doyle said: “It is possible that there are other victims of Levy who have not yet reported what happened to them. If you recognise Levy and believe you may have been a victim of his offending, I encourage you to come forward and report it to us…”The Met, British Transport Police and the Crown Prosecution Service have all admitted failings in how they dealt with Levy. He will be sentenced on Wednesday.
