Investigating Directorate Against Corruption (IDAC) head Andrea Johnson blamed her chief investigator for a critical error that widened the unit's investigation into senior Crime Intelligence officials beyond the original corruption complaint.

Testifying at the Madlanga Commission on Thursday, Johnson said on Thursday that IDAC chief investigator Dylan Perumal was responsible for adding extra names to the investigation before she authorised it.

This led to the complaint against three senior Crime Intelligence officers transforming into a full-blown probe involving eight senior officers and a civilian.

The investigation originated from a corruption complaint lodged by National Coloured Congress (NCC) MP Fadiel Adams, whose submission identified only three officials.
