Ian Coates, Barnaby Webber, Grace O’Malley-Kumar. Their names are instantly recognisable for the most terrible of reasons.

They were stabbed to death by paranoid schizophrenic Valdo Calocane in Nottingham three years ago. He is currently serving an indefinite hospital order after pleading guilty to manslaughter on the grounds of diminished responsibility.

Meanwhile, the families of the victims have been fighting for answers about what went wrong: Why Calocane, a man with a history of violence as well as serious mental health problems, was free to kill?

Evidence to the public inquiry which has just concluded was painfully familiar. Missed opportunities, failure to assess risk, no one talking to each other, the language of inquiries spanning decades. The report will be published next year.
