For many patients, a cancer diagnosis brings not only medical decisions, but also a complex experience that includes understanding treatment options, coordinating appointments, managing side effects, and balancing everyday responsibilities. Through ongoing outreach and coordination with care teams, patients receive guidance to understand their treatment plans, access supportive services, and stay on track with care, helping reduce delays and prevent avoidable complications.As part of this approach, Cancer Care Engagement (CCE) uses predictive analytics to identify patients at higher risk for complications and connect them with certified oncology navigators who serve as a consistent point of contact. Navigators help coordinate care, answer questions, and connect patients to services such as behavioral health, transportation, and pharmacy support. Among patients reached, CCE has achieved an 84% engagement rate and has been associated with a 7% reduction in avoidable inpatient admissions for Medicare Advantage members and 15% for commercially insured.CCE also offers digital care management support designed to deliver a highly personalized experience, including education, symptom checkers, integrated remote patient monitoring, and chat capabilities with care navigators. Taking a whole-person approach, the program addresses both clinical and day-to-day needs through tailored care plans, education, and access to a dedicated care team via digital and telephonic channels. Ongoing outreach and 24/7 support provide guidance when it’s needed most, from managing side effects to coordinating care. This approach has contributed to a 66% reduction in hospital admissions for chemotherapy-related side effects and a 25% reduction in 30-day readmissions among recently discharged members.Should advanced-stage illness support be needed, Elevance Health can also connect patients to additional care management resources designed to address complex medical, behavioral, and psychosocial needs. This includes personalized care planning and coordination through interdisciplinary clinical teams to support patients throughout their care journey.In addition, Elevance Health works closely with oncology providers to support more coordinated, high-quality care. Oncology Provider Enablement Clinicians (OPECs) partner with oncologists and use data-driven dashboards to track key metrics and pathway adherence, helping improve treatment consistency, reduce drug waste, and lower emergency room visits.Together, these capabilities help create a more connected, patient-centered care experience.
