Free contextual chunk headers: heading-aware chunking for hybrid retrieval

A deterministic alternative to Anthropic's contextual retrieval. Prepend the markdown heading breadcrumb into each chunk's text before embedding; both the vector and the BM25 tsvector pick it up.
