The assessment named three April 2025 attacks as the sources of the civilian casualties, and noted that 15 other incidents were still under review, per the reports. The Pentagon’s analysis focused on military operations in 2025, and did not address potential casualties tied to the U.S.’s 2026 strikes on Iran.
