Community Care Cooperative (C3) Delivers $4.1 Million in Shared Savings Through ACO REACH in 2024
Community Care Cooperative (C3), the largest non-profit, FQHC-governed Accountable Care Organization (ACO) in the country, announces it has earned $4.1 million in shared savings through the ACO REACH Model for Performance Year 2024. Our success was driven by generating $8.4 million in gross savings for Medicare, representing a 4.8% gross savings rate, underscoring C3’s ability to deliver measurable value through efficient care coordination and innovative strategies that improve outcomes for thousands of patients, while lowering costs.
The results reflect C3's continued investment in value-based care, responsible financial stewardship, and close collaboration with community health centers. By combining data-driven population health strategies with strong clinical partnerships, C3 continues to demonstrate that higher-quality care and lower costs can be achieved together.
A key driver of this success is C3’s partnership with Federally Qualified Health Centers (FQHCs). Community health centers play a critical role in delivering comprehensive, accessible care to patients, ensuring that preventive services and chronic condition management remain a priority. By supporting FQHCs with resources and tools for value-based care, C3 strengthens local care delivery and helps patients receive the right care at the right time.
