The NHS is bracing for a report on the biggest maternity scandal in its history, which is due to be published tomorrow.

An inquiry led by senior midwife Donna Ockenden looked at 2,500 cases involving babies and mothers who died or were injured while under the care of Nottingham University NHS Trust, and her findings are expected to make for harrowing reading.

We spoke to Jack and Sarah Hawkins who have campaigned tirelessly since the death of their daughter Harriet.
