Clarify Health (“Clarify”), a tech-enabled outcomes company delivering referral intelligence, quality measurement, and network performance solutions for health systems, today announced it has completed the acquisition of Loyal Health Holdings, Inc. (“Loyal”), a healthcare-specific patient activation platform. The combination creates the industry’s first closed-loop network intelligence engine, a single platform that identifies where patients need to go for care, activates them into the right provider, and measures whether it worked.
